Size 45.0 Baruntses in great shape. Only used on about 4 weekend-long ice climbing trips. These are just a hair small for me, which aggravates my big toe's toe nail on both feet. I can answer all of your sizing questions, as I have multiple La Sportiva and Scarpa boots I can use as a reference. The inner boots, which have never been heat molded, are in excellent shape.
